The "Medical Home" Model Explained

The term "patient-centered medical home" probably conjures up a number of images and ideas. 

It is not a home you LIVE in, but a home base of sorts, a place to get all of your medical needs addressed in one place. 

The concept is catching on in Oregon. 

Grants Pass physician Shae Johnson and practice manager David Gilmore join us to explain the benefits of the "medical home."
